Forms for Domestic Students
Application and Checklist
The following checklist identifies required documentation for students who have completed 100 hours of classes in an American university.
1. Complete all forms in ink or by typing. Sign in ink and date where required. A link to application instructions may be found above.

A bank check or money order made out to Southern Illinois University and mailed to the department may be sent in if you are not able to pay by credit card with the online application. - Personal

Biographical Sketch
- (Required documentation if you are nominated by the committee for a fellowship. The following is found on the Graduate School website of application instructions for fellowships: Biographical statement (one typed page) addressing your academic qualifications in regard to academic excellence which should include, but not limited to the following: academic achievements, honors, awards, publications, and extracurricular activities.)

Personal Statement (Evidence of commitment to graduate study)
- (Required documentation if you are nominated by the committee for a fellowship. The following is found on the Graduate School website of application instructions for fellowships: Personal Statement (one or two typed pages) indicating your personal experience, educational goals and professional interests and how these items can contribute to a more reflective, responsive, environment in the program, the discipline and the larger university community).
For each COLLEGE and/or UNIVERSITY attended**:

- Official Diploma(s)
**Official
copies of the final transcripts and diplomas will be required by the
SIUC Graduate School before first registration for successful candidates.
3. It is recommended to send the following documentation:
- GRE - Photocopy of student report, if taken.
Depending on individual circumstances, there may be additional requirements that must be met.

Application and Checklist
The following checklist identifies required documentation for students who do NOT have English as their native language or who have NOT completed 100 hours of classes in an American university.

2. Send the following documentation:
- Passport Page (copy of passport page with name and date of birth, required for processing application)
- TOEFL - Photocopy of student report required for departmental application process
- Official ETS report is required by Grad School if applicant selected and referred by dept.
- Exam must have been taken with 24 months of first registration
- Grad School requires minimum TOEFL score of 550 (written exam) or 220 (computer exam)
For
each COLLEGE and/or UNIVERSITY attended**:

- English translation
If you are currently enrolled but will not be receiving a degree, make sure current and final transcripts will be available.
For each DEGREE received**:
- Diploma - Official native-language
- Diploma - English translation
**Note: Final transcripts and/or diplomas will be required by Graduate School before first registration for successful candidates. This requirement will be noted on an I-20.
3. It is RECOMMENDED to send the following documentation:
- GRE - Photocopy of student report, if taken.
